			<content:encoded><![CDATA[<p><img src="http://www.penis-resources.com/blog/images/journal-electric.jpg" width="171" height="195" class="imgRight" alt="JSM Electric Shock" />Each and every year the understanding and awareness of male sexual dysfunction problems increases. As medical science advances in the pursuit of solutions for issues such as erectile dysfunction, new methods for finding a viable cure are constantly explored. One relatively new type of treatment which involves the use of electrical impulses has already shown favorable results as documented in a recent study published by the Journal of Sexual Medicine.</p>
<p>We first reported about this innovative new therapy back in December of 2009 in an article entitled &quot;<a href="http://www.penis-resources.com/blog/sexual-dysfunction/electric-shock-treatment/">Electric Shock Treatment for ED</a>&quot;. Although documentation regarding this type of erectile dysfunction treatment remains sparse, we did manage to uncover some research which seems to add credibility to this method. The aim of the following <a href="http://onlinelibrary.wiley.com/doi/10.1111/j.1743-6109.2010.02103_1.x/abstract" rel="external">published study</a> was to measure the effect of low intensity shock wave therapy in severe ED patients not responding to oral medication.</p>
<blockquote><p><strong>Objective:</strong> Low Intensity Shock Wave Therapy (LI-ESWT) was shown to have a beneficial effect on ED patients responding to PDE5i's. This study aimed to assess its effect on PDE5i's non-responders.<br />
<strong>Methods:</strong> We included severe ED patients (60&#37; diabetic, 85&#37; with cardiovascular disease) that failed to respond to PDE5i therapy, who scored 0-2 on the rigidity scale (RS) during PDE5i therapy. Each subject underwent a full baseline assessment of erectile and sexual function (during PDE5i treatment) using validated questionnaires and objective penile endothelial function testing. Treatment included 2 sessions/week for 3 weeks, repeated after a 3-week no-treatment interval. At each session LI-ESWT was applied on the penile shaft and crus for 3 minutes in 5 different penile anatomical sites (intensity of 0.09 mj/mm2, 300 shocks/site). One month after end of treatment the same baseline assessment was repeated. An active PDE5i medication regime was then provided and final erectile function was reassessed. Main endpoints for success were changes in RS and in IIEF-ED Domain score (EDDS).<br />
<strong>Results:</strong> Twenty-four subjects age 40-78 (mean age 62.8) with an initial average EDDS of 8.8 +/- 0.99 (on PDE5i therapy) were analyzed. All completed a full 12 week treatment course. After one month their EDDS significantly increased to an average of 13.6 +/- 1.11 (without additional medication). At the end of the active PDE5i treatment the mean EDDS was 20.07 +/- 1.33 (an increase of 11.27 points, p < 0.001) and 80&#37; of the patients had a RS of 3 and more (p < 0.0001). Five of 15 subjects were normalized with use of PDE5i. All FMD parameters significantly increased and no adverse events were reported.<br />
<strong>Conclusion:</strong> LI-ESWT is a new modality that can be used to effectively treat a subgroup of severe ED patients who failed PDE5i therapy. This study further emphasizes the physiological effect that LI-ESWT can have on the erectile mechanism.</p></blockquote>

<h2 class="title">Advances in Treatment for SEVERE ED</h2>
<p>While the results of this study are promising, it should be noted that the treatment is intended for <em>severe cases</em> of erectile dysfunction such as is sometimes seen in men suffering with diabetes and/or cardiovascular disease. Many times these men do not respond positively to more traditional methods that involve the use of PDE5i's (Phosphodiesterase Type 5 Inhibitors) such as Sildenafil (Viagra&reg;), Vardenafil (Levitra&reg;) and Tadalafil (Cialis&reg;).</p>
<p>This revolutionary new technique represents a potential breakthrough in restoring erectile function in those men who could not otherwise be helped. It will be interesting to see where future research into shockwave therapy for the treatment of erectile dysfunction leads. The cure for tens of thousands of severe ED sufferers may be just around the corner!</p>

While there have been a number of advances in the understanding of this particular type of sexual dysfunction, many researchers still debate the exact underlying causes. A recent study published in the British Journal of Urology explores the possible connection between premature ejaculation and penis sensitivity.</p>
<h2 class="title">The Sensitive Penis</h2>
<p>The human penis is an intricate structure composed of tissues, veins and skin. The penile skin contains many sensitive nerve endings that respond to touch and cause sexual arousal resulting in an erection. While this reaction is to be expected in a normally functioning penis, some have questioned whether it's possible that some men are <em>too sensitive</em> to stimulation, or hypersensitive, which results in early ejaculation.</p>
<p>The British Journal of Urology recently published the results of a <a href="http://www3.interscience.wiley.com/journal/123523259/abstract" rel="external">study</a><sup><nobr>&dagger;</nobr></sup> investigating the correlation between penile hypersensitivity and premature ejaculation (PE).</p>
<h2 class="title">Penis Sensitivity Study</h2>
<p>The basis for the belief that penile hypersensitivity may be a cause of PE stems from earlier clinical trials. These studies demonstrated the effectiveness of certain topical desensitizing agents (PSD502) in prolonging the ejaculatory response. While the use of such local anesthetics has been shown to help delay ejaculation, the results of this new study casts some doubt on the hypothesis that penis sensitivity is the leading contributor of PE.</p>
<p><strong>Results</strong></p>
<blockquote><p><em>&quot;The published data does not support unequivocally penile hypersensitivity as the cause of PE. Interpretation of the data is hampered by the variability of the populations described as having PE across studies. Data from the PSD502 clinical trials clearly shows that PSD502 increases ejaculatory latency, and improves control and sexual satisfaction when applied topically to men with PE 5 min before intercourse, enabling subjects to delay ejaculation up to six times longer than those who used a placebo.&quot;</em></p></blockquote>
<p><strong>Conclusion</strong></p>
<blockquote><p><em>&quot;The clinical profile of PSD502 lends credibility to the penile hypersensitivity hypothesis for PE. The predominant action of local anaesthetics is to reduce neuronal firing in sensory afferents; the clinical profile of PSD502, which shows improvement of ejaculatory function in the absence of a generalized reduction in penile sensitivity, can most readily be explained based on an underlying hypersensitivity in patients with PE.&quot;</em></p></blockquote>

<h2 class="title">Hypersensitivity &#8211; ONE Cause of PE</h2>
<p>What this study basically finds is that penile hypersensitivity may be a contributing factor to premature ejaculation but it is probably not the only one. Medical experts now generally agree that there are <a href="http://www.penis-resources.com/blog/sexual-dysfunction/different-types-premature-ejaculation/">many types of PE</a> and it is highly unlikely that any one cause is entirely responsible for the condition.</p>
<p>This should not detract from the effectiveness of certain treatments, such as the one mentioned in the study, that have shown promise in the treatment of PE. It merely means that there is no one cause, and therefore no single solution for all sufferers of premature ejaculation.</p>

Most men will experience this condition at least once in their lifetime and some estimates suggest that between 25&#37;-40&#37; of men in the United States alone suffer from it regularly. While until recently premature ejaculation was commonly attributed to either psychological or physiological factors, new theory calls for a greater integration of these two elements and supports an expanded range of classification for this common male sexual condition.</p>
<h2 class="title">The Four Classifications of PE (Premature Ejaculation)</h2>
<p>Although <a href="http://www.penis-resources.com/premature_ejaculation.html">premature ejaculation</a> is a frequent sexual complaint from males, the occurrence of PE is not always the result of purely mental or physical factors. A newly proposed classification system separates the condition into four specific syndromes; lifelong PE, acquired PE, natural variable PE and premature-like ejaculatory dysfunction.</p>
<p>Each syndrome is distinguished by certain specific variables:</p>
<ul>
<li>Duration of IELT (Intravaginal Ejaculation Latency Time) &#8211; this is the amount of time it takes from actual vaginal penetration until ejaculation occurs</li>

<p><strong>Lifelong PE</strong><br />
For men with lifelong PE, ejaculation usually occurs within 1 minute of penetration. Men under this classification have experienced premature ejaculation starting with their very first sexual encounter and the condition has persisted ever since. While much research has been done in recent years to determine the underlying causes of erection problems, there has been little evidence to support a direct link between this condition and the size of a penis &#8211; that is, until now. A new study suggests that there may in fact be a direct correlation between penis size and erectile dysfunction.</p>
<h2 class="title">Case Study</h2>
<p>A study<sup><nobr>&dagger;</nobr></sup> recently published in the Journal of Sexual Medicine was conducted to determine the average stretched penis size in normal men compared with that of patients suffering from erectile dysfunction. The test group consisted of over one thousand subjects from a hospital outpatient clinic and contained both potent males and those suffering with ED. Using a tape measure and ruler, both penile length and girth in the fully stretched states were measured to determine average size.</p>
<p><strong>Results</strong></p>
<p>In the group of normal men with no symptoms of ED, it was determined that the average fully stretched length was 12.9 cm (+/- 1.9 cm) and the fully stretched girth was 8.9 cm (+/- 0.9 cm). In the group with ED patients, the average fully stretched penis length was 11.2 cm (+/- 1.5 cm) and fully stretched girth was 8.8 cm (+/- 0.8 cm).</p>
<p>Using the data compiled it can be seen that while girth measurements between the two groups were fairly consistent (only a 0.1 cm difference), the length differential was quite pronounced. Those men not suffering from erectile dysfunction were approximately 1.7 cm larger &#8211; or nearly .67 inches longer in size. These statistics suggest that men with ED tend to have <em>significantly shorter penises</em> than those without erection problems.</p>

<h2 class="title">Why Do Men with ED have Shorter Penises?</h2>
<p>While the researchers themselves offer no suggestions as to why this discrepancy in size exists, I suspect it is tied to both physical and psychological reasons.</p>
<p>Physiologically speaking, the penis tends to shrink in size when it is not functioning properly. Without adequate and regular blood flow to the muscles, veins and arteries in and near the corpora cavernosa, the surrounding tissues of the penis breakdown and atrophy.</p>

To my knowledge there have been no official studies performed to calculate the exact size of the male population suffering with this condition. Given the sensitivity of the subject, I'm not entirely certain that such a census could even be accurately conducted. There is however an unofficial study that was recently published in Men's Health magazine that attempts to rank the frequency of erectile dysfunction within 100 U.S. cities based on several specific factors.</p>
<h2 class="title">Criteria for Ranking Erectile Dysfunction Frequency</h2>
<p>To determine the likelihood of occurrence per city, the following criteria was examined:</p>
<ul>
<li>The rates of three known risk factors &#8211; smoking, obesity and diabetes.</li>
<li>The percentage of the male population that performs regular exercise.</li>
<li>The number of urologists per capita.</li>
<li>The number of Viagra, Cialis and Levitra prescriptions filled.</li>
</ul>
<p>Using this available information, the following table attempts to grade each city based on a scale of A through F.</p>
