As many men are unfortunately aware, premature ejaculation is a sexual dysfunction in which a man ejaculates earlier than either he or his partner would like. Most men will experience this condition at least once in their lifetime and some estimates suggest that between 25%-40% of men in the United States alone suffer from it regularly. While until recently premature ejaculation was commonly attributed to either psychological or physiological factors, new theory calls for a greater integration of these two elements and supports an expanded range of classification for this common male sexual condition.

The Four Classifications of PE (Premature Ejaculation)

Although premature ejaculation is a frequent sexual complaint from males, the occurrence of PE is not always the result of purely mental or physical factors. A newly proposed classification system separates the condition into four specific syndromes; lifelong PE, acquired PE, natural variable PE and premature-like ejaculatory dysfunction.

Each syndrome is distinguished by certain specific variables:

Duration of IELT (Intravaginal Ejaculation Latency Time) – this is the amount of time it takes from actual vaginal penetration until ejaculation occurs

Frequency of PE complaint

How PE progresses over time

The underlying cause of the disorder [Etiology]

The actual physiological changes that occur due to the disorder [Pathophysiology]

Recommended Treatment of PE

Lifelong PE
For men with lifelong PE, ejaculation usually occurs within 1 minute of penetration. Men under this classification have experienced premature ejaculation starting with their very first sexual encounter and the condition has persisted ever since. It is believed to be a mainly biological disorder and the use of drug treatment is required to delay ejaculation.

Acquired PE
Acquired PE, also called secondary premature ejaculation, describes a condition in which PE did not always exist but developed at some point in time. The cause may be either physiological or psychological (or both). Suggested treatment involves specific counseling, or in the case of an actual biological disorder such as thyroid or urological disease, medical treatment may be necessary.

Natural Variable PE
Men with natural variable PE only experience early ejaculations occasionally. Since there is no indication that an occasional occurrence of premature ejaculation is either related to psychological or physiological disorder, it is assumed that this condition is only a manifestation of normal ejaculatory performance. The treatment for those men who fit into the natural variable PE classification is generally limited to counseling.

Premature-like Ejaculatory Dysfunction
Premature-like ejaculatory dysfunction is the classification for those men who complain of rapid ejaculation yet have normal IELT (3-7 minutes) or even longer IELTs (10-25 minutes). Since these IELTs adhere to what is largely considered normal intercourse duration time and there is no evidence of physiological disturbance, it is very likely that this syndrome has either psychological or cultural origins. For men experiencing this condition, treatment with medicine is not recommended. Instead, counseling or psychological therapy designed to help them cope with their complaints should be pursued.

In a recent study† to determine the prevalence of these four PE syndromes among patients admitted to a urology outpatient clinic, it was concluded that the majority of patients who seek treatment for premature ejaculation fit into the classification of lifelong PE. It should be noted that the results of this particular study only suggest that most men who actually seek treatment are classified as lifelong PE sufferers. The percentage of men correlating with the other three premature ejaculation syndromes and who never seek treatment may be significantly higher.

Source:†Journal of Sexual Medicine / DOI 10.1111/j.1743-6109.2009.01570.x

Dealing with Premature Ejaculation

The new classification system for PE reveals that are many different variations of premature ejaculation to consider. Some instances of PE are caused by an actual biological disorder and require medical attention while others are more likely related to psychological issues. Regardless of the exact root cause of the condition, the end result is ultimately an undesired effect on male sexual performance.

In many cases where men feel they're not performing to their full ability and would like to increase the length of time before ejaculation occurs, there are simple techniques available that can help improve IELT. Penis exercises for instance, specifically those that target premature ejaculation, provide an easy and inexpensive way to help increase sexual endurance.

Another excellent method used to combat PE is through the use of a specially designed penile traction device. These devices can help prevent premature ejaculation and also offer the added benefit of increasing overall penis size.

While these techniques are not meant to replace medical treatment in those cases where an actual physiological pathology exists, they can be helpful for the large percentage of men who simply wish to prolong intercourse through better ejaculation control. As always, it is best to consult a physician for advice if you believe the severity of your condition warrants medical attention.

Recently a new "cure" for premature ejaculation has been making some headlines. Basically it's a topical anesthetic spray that when applied to the penis will cause a numbing effect. The general concept behind this idea is that if there is less sensation in the penis, ejaculating too quickly is less likely to happen. But is effectively putting your penis "on ice" the way to go?

Being somewhat obsessed with male enhancement and all things Penis (or a "Dick-Head" as my girlfriend affectionately refers to me), when I saw a link to an article entitled "Topical Spray May Treat Premature Ejaculation", I just had to check it out. The information truly wasn't earth-shattering. This kind of treatment has in fact been around for quite some time, most notably with some types of condoms that are supposed to delay ejaculation.

While I personally have never used any type of numbing process to prolong coitus, I did remember a somewhat humorous story told to me by a woman I used to work with. She told me that both her and her husband had been a bit upset over the fact that most of their sexual relations seemed to be over just about as quickly as they began. This is not surprising since it's been reported that the rate of premature ejaculation in American males is estimated to range from 30-70%.

While shopping one day the husband came across some condoms that were supposed to help you last longer during sex so he immediately bought a few dozen. After he arrived home, he told his wife what he had done and they decided to give it a shot. So the husband puts on the condom, proceeds to have sex with his wife and after about 30 seconds he ejaculates. He's quite upset since he was expecting to be a "manly man" and give his wife the pounding of her life.

Well about 15 minutes goes by and he starts to feel a little funny… his penis has lost sensation! He begins to panic a bit (who wouldn't?!) until his caring and thoughtful wife decides to actually read the small print on the condom box. Thats when they discover that the condoms are supposed to numb the penis and that they should be worn approximately 15 minutes prior to intercourse in order for the process to take effect.

Ok, maybe I should have felt more sympathetic for this guy but I have to tell you that when she told me that story I almost broke a nut. I could imagine this poor fellow, sitting there after yet another "sex sprint" with a useless penis hanging between his legs. His wife really didn't help my disposition either since she was almost in tears from laughing while she told me about that episode.

I just don't understand how this concept is a viable option for men suffering from premature ejaculation. Firstly, there's the waiting period before it actually takes effect. I say that when the time is right, there's no room for stalling. But ok, maybe you spend a little time with foreplay and wait until your penis is dead… errr "numb." Now what? The best thing about sex is the pleasure you feel while doing it! If you can't feel a damn thing then what good is that? Maybe the wifey is having the time of her life but sex is a two way street – if both partners aren't experiencing pleasure I just can't understand the point.

Another thing that bothers me is the actual process itself. Like most guys I love my penis. I take care of it, I groom it, I talk to it (they say it helps make it grow) and I certainly don't want to apply some "anesthetic" that is going to give it rigamortis! Not to mention the possible effects it might have on the inner workings of a woman's vagina. I don't think most women would like their girlie-parts on ice either.

Yes, premature ejaculation is a common problem that some reports say effects every 1 out of 2 men at least once in their lifetime. And while the controversy rages over whether the cause is mostly psychological, physical, or both, I say that men need to get more in touch with their "inner penis."

Maybe it's because I'm overly obsessed with my own genitalia, or that I exercise my penis more than most health nuts exercise their biceps in the gym, but I can honestly say that I do not suffer from this problem. Heck, I usually even have to concentrate in order to just ejaculate (after about an hour, even the most stalwart of sexual partners usually want a break).

Another factor I believe influences this condition is that men are self-absorbed. It's all "me, me, me" when it really should be about "us." Most men get so excited about the prospect of sex that they're like kids on Christmas eve. Well if you get too excited then Santa just might be showing up earlier than you expected with a sack full of something other than toys.

Numbing your penis is not the way to go. Try to remember that sex is supposed to be a wonderful experience between two people (maybe more, depending on your tastes) and that pleasure is not just what you feel, but what you give. If you want to put something on ice, start with the other head first!
